To be able to read and print the forms, you need to have Adobe Reader installed on your computer. For best results, we recommend version 6 or later. The programme is free and can be downloaded from the Adobe website.

The programme does not allow you to save a completed PDF form as a file or to send it to us by email. If you wish to use these facilities, you can read about what is required on the Adobe website.
